2010-2011 REGISTRATION
Please print a copy of the Registration Application and mail your completed FORMS with payment to the
TROY ALBANY HOCKEY ASSOCIATION, PO Box 294, Wynantskill, NY 12198
PAYMENT - Payments may be made by check or credit card. The league accepts MasterCard and Visa. Checks may be made payable to the Troy Albany Hockey Association.
CREDT CARD TRANSACTIONS – Please be advised that the league will NOT process credit card transactions on an automated basis or on pre-scheduled dates. “ALL” credit card payments will require you to EITHER complete and submit the Credit Card Authorization Form OR you may contact Sue Goldfarb directly at ggoldfa1@nycap.rr.com / 518-279-1305 to make credit card payment arrangements.
FEE SCHEDULE -
AAA, A, B, C - Registration is due in two equal installments, July 1 and October 1.
Learn to Skate, Cross Ice, MITES- Registration is due October 1.
MIDGET 19U/16U & ICE CATS - Registration due in two equal installments. 1st install date TBA after tryouts, and October 1. Tryout dates TBA.
* All fees are to be paid in full by October 1.
NON REUNDABLE - Players that make teams at the travel level only (AAA, A, B and ICE CATS 12U-19U), a $500 non refundable down payment is due at the completion of tryouts in order to secure your players spot on that team.
ICE CATS - "A and B level rates apply" to those ICE CAT players chosing to play on, or who are rostered with both boys (A or B) level teams and the ICE CAT's.
USA HOCKEY REGISTRATION - Every player must register with USA Hockey online. Your registration is NOT complete until this is done and your player will not be allowed to participate until we have received his/her USA Hockey confirmation code. Please visit the registration page on the USA Hockey website http://www.usahockeyregistration.com or follow the USA HOCKEY REGISTRATION INSTRUCTIONS link. Upon completion you must e-mail the confirmation code to our registrar Martha Mainello at kevinjcase1150@aol.com. The USA Hockey registration fee of $33 is not included as part of the leagues registration fees.

FORMS TO BE SUBMITTED WITH YOUR REGISTRATION APPLICATION
USA HOCKEY CONSENT TO TREAT FROM: This needs to be completed for every "Player" and "Coach" each season and travel with the Coach of every team. Required documentation for NY State Sectionals and State Tournaments.
USA HOCKEY PARTICIPANT CODE OF CONDUCT: This needs to be completed for every "Player" each season and kept on record by the Coach or Team Manager of every team. Required documentation for NY State Sectionals and State Tournaments.
NYSAHA RELEASE FORM: This is required when registering with Troy Albany Hockey having played elsewhere the previous season. This needs to be completed and signed by your current association president before trying out for a TAH team. Required by New York State Amateur Hockey and the Troy Albany Hockey Association.
NYSAHA COMMITMENT FORM: This needs to be completed for every "Player" registering with the Troy Albany Hockey Association.
